Amosup Dependent ID to be updated

Recently me and my wife went to amosup hospital for a check-up,the procedure was still quite
good.The staff were still accomodating.If you are an amosop member and have a registered dependent
your dependents ID will updated and here are the requirements;
For single member(Parent,Brothers,Sister)
1.Brown long envelope(write last name,first name and contact no.
2.Latest allotment slip
3.AMOSUP ID#
4.Member Data Record(MDR)from philhealth(for seaman all dependents must be listed in either MDR
5.1x1 Picture for the seaman
6.All dependents must take their pictures on site via webcam
7.Original NSO of no marriage(Seaman)
8.Orinal NSO Birth Certificate of seaman(1 zerox)
9.Original NSO Marriage Certificate of Parents(1 zerox)
10.Original NSO Birth Certificate(Brother and/or siter(1 zerox)
If the parents are over 60
1.Senior citizen ID Original & 1 copy back & front
2.Notorized Affidavit of Support
Spouse & Child( for Married Seaman)

8.Original NSO Birth Certicate of children & 1 copy
if seaman or spouse are over 60
9.Senior Citizen ID Original & 1 copy back & front
Children of single parent
